Population Health

South King Street Medical Centre – Population Health

The first joint project for the primary care network was population health management – a pilot project to see if we can change the health of patients by improving their social issues. We have started to look at a specific group of patients who live in houses of multiple-occupancy. If this pilot session works, it will be rolled out across the Central West Network. Have a look at the video below which shows the presentation given by members of the Central West Network Team. Here they explain the project, how far we’ve got, and how far we’ve got to go.

As part of this project we worked very closely with the Central West Neighbourhood team. Health & Wellbeing worker, Helen Jenkins, played an integral part in delivering support to the patients we had identified for this project. The link below will take you to a blog written by Helen who describes some of the challenges faced by those patients.
