South King Street Medical Centre – Population Health

The first joint project for the primary care network was population health management – a pilot project to see if we can change the health of patients by improving their social issues. We have started to look at a specific group of patients who live in houses of multiple-occupancy. If this pilot session works, it will be rolled out across the Central West Network. As part of this project we worked very closely with the Central West Neighbourhood team. Health & Wellbeing worker, Helen Jenkins, played an integral part in delivering support to the patients we had identified for this project.
